Payment must be received prior to appointment unless otherwise agreed.

Clients agree to provide medical and GP information; consultation details and plans will be shared with clients GP or medical team unless otherwise agreed. 

The responsibility for clients kidney care lies with their medical team and is not the responsibility of the dietitian. 

Client accepts that kidney health may worsen despite dietary changes.

The dietitian does not accept responsibility if client experiences a decline in health if dietary advice is not appropriately followed.
